To start the process of placing a child you must: 

Call our office and answer prospective child application questions over the phone (660)347-5982

Fax (660)347-5711 or mail to the above address a Psychological Evaluation or Assessment of the child

This information will be reviewed with the child intake team and they will contact you to set up a pre-placement interview or with other options for your child.  A decision to accept your child will be made following the pre-placement interview. 

If your child is accepted, you will need to bring with you the items listed below:

Court Order of Placement (if applicable)

Divorce Decree or Custody Order (if applicable)

Original Social Security Card

Original Birth Certificate

Original Insurance or Medicaid Card

Current Medications or Prescriptions (if applicable)

Immunization Records

IEP (if applicable)

Physical
